Allied Benefit Systems Claims Mailing Address
Most Allied Benefit Systems claims go electronically to payer ID 37308. Paper still matters for some situations, including corrected claims with attachments, member-filed claims, medical records and appeals. This guide gives the claims mailing address Allied currently publishes, explains when to use the address on the ID card instead, and lists what to put in the envelope.
Which address to use
Allied's medical claim form does not print a fixed claims address at the top. It says to use the address on the back of the employee's ID card. The fax cover sheet and the dental claim form both print P.O. Box 211651 in Eagan, Minnesota. Allied's contact page lists the same P.O. Box for mailing documents related to a benefits plan.
So the address has two layers. The card is the plan-specific instruction and always comes first. P.O. Box 211651 is the general address Allied uses on its own forms right now, and it is the right fallback when the card simply shows Allied's standard address. If a card prints a different P.O. Box, it may belong to a particular client program or network arrangement. Use the address on the card.
Two addresses not to use
200 West Adams St., Suite 500, Chicago, IL 60606 is Allied's corporate office. Allied's contact page says it does not receive mail. P.O. Box 909786, Chicago, IL 60690 appears on at least one older third-party plan directory but not on Allied's current forms. Use it only if a member's card prints it.
USPS versus overnight carriers
A P.O. Box cannot accept FedEx or UPS deliveries. That is why Allied's contact page gives a separate street address for those carriers:
| Carrier | Address | Good for |
|---|---|---|
| USPS (first class, certified) | Allied Benefit Systems P.O. Box 211651 Eagan, MN 55121 | Routine paper claims, corrected claims, records requests |
| FedEx or UPS | Allied Benefit Systems 960 Blue Gentian Road Eagan, MN 55121 | Time-sensitive appeals, large record sets, anything that needs signed proof of delivery |
If you are close to a filing deadline, a tracked delivery gives you a dated receipt. Allied's claim form says late charges are denied, and you may need that receipt to show when the claim arrived.
Faxing claims and documents
Allied publishes a claim submission cover sheet with fax number (312) 906-8359 and phone (800) 288-2078. The sheet asks for the employer name, group number, employee name and unique ID, patient name, date of service, claim number and provider name. It also has check boxes for the type of document you are sending:
- Medical record
- New claim(s) or corrected claim(s)
- Repricing or repricing sheet
- Coordination of benefits letter
- Itemized bill
- Explanation of benefits (EOB)
- Other, with a description
Use a separate cover sheet for each patient and claim. A fax that mixes several patients is hard to index, and the second claim in the stack is the one most likely to get lost. You can download the sheet from Allied's provider documents page.
Paper claim checklist for providers
- Use the right form. Use a CMS-1500 (02/12) for professional services, a UB-04 for facility claims, and the ADA form for dental. Allied's own medical claim form is mainly for members filing their own claims.
- Copy the member ID and group number exactly from the card, including any prefix or leading zeros.
- Include your NPI and tax ID, and make sure the billing name matches your W-9. Allied's provider documents page has its own W-9 if you need to give a payer W-9 to your office.
- Attach other-payer EOBs when Allied is secondary. Without them, coordination of benefits will hold the claim.
- Mark corrected claims clearly. Put the frequency code in box 22 of the CMS-1500 or the third digit of the type of bill on the UB-04. Include the original claim number too. See the corrected claims and appeals guide.
- Keep a copy and the mailing date in the patient account.
When members file their own claims
Members who paid a provider directly can file with Allied's medical, dental, vision or international medical claim forms. You will find them on Allied's member resources page. The medical form lists what an itemized bill must show. That includes the provider's name, address and tax ID, the date of service, diagnosis and procedure codes, and the charge for each service. The form asks members to sign the authorization and, if they want payment sent to the provider, the assignment of benefits. Members mail the form and the itemized bill to the address on the back of the ID card.
If a patient asks your office where to send their own claim, point them to the card and to Allied's customer service number. Do not guess. Allied's contact page notes that members should call the number on the back of their card to reach the team assigned to their plan.
Why electronic is usually better
Paper takes longer to arrive and to be entered by hand, and it gives you less tracking. Allied's provider page says sending electronic claims through a clearinghouse helps providers get paid faster. Keep paper for the cases that need it. If you are setting up electronic billing for the first time, the 837P and 837I submission guide explains clearinghouse setup with payer ID 37308. The eligibility verification guide covers checking coverage before the claim goes out.
